Sherburne Commons currently works directly with TUCKedIn Elder Care and Best of Care for emergency response services, check-ins, and call system monitoring. They are a home care agency that also provides Geriatric care management services, caregivers and home health aides. They hold an office at Sherburne and operate the wellness center which is available to all Residents for consultation, assessments, care planning, care management, immediate first aid, routine blood pressure and weight checks, and referrals to community health services. Services can be provided and tailored to your individual unique needs so you only pay for what you need when you need it. In this way, Best of Care can make daily life easier for our residents by offering the desired service and care as needs and preferences dictate and have easier access to them by being onsite.

As our preferred provider and emergency response service, Best of Care also provides a complimentary initial assessment and at least one assessment each year to Residents. This assessment would be done by a Registered Nurse with Best of Care. If it is determined that any additional services or care may need to be provided, the reason and suggestion for what care will be communicated with the Resident for their review. The Resident can then engage the services of Best of Care or any agency of their choice. Care Managers with TUCKedIn or Best of Care as well as the management at RSC can assist you with understanding what providers are available based upon your needs and the management team at Sherburne will work with you to ensure integration of our preferred provider into our community.
